I, wishing to volunteer my time and services for the Louisville Nature Center, hereby acknowledge that volunteering may put me at higher risk of exposure to Novel Coronavirus, or COVID-19. I hereby assume the risk of bodily injury, illness, death, medical treatment resulting from my volunteer activities even if resulting from the negligence of the Louisville Nature Center or its employees. I hereby release, discharge and agree to indemnify and hold the the Louisville Nature Center harmless from, and waive on behalf of myself, any and all causes of action, claims, demands, damage, costs, expenses and compensation or loss to myself that may be caused by any act, or failure to act of the the Louisville Nature Center, or that may otherwise arise in any way in connection with any voluntary activities with, or for the the Louisville Nature Center. I agree to follow Centers for Disease Control (CDC) and local health district guidelines and the Louisville Nature Center policies and procedures for social distancing to reduce the spread of COVID-19. This will require me to maintain six (6) feet of distance between myself, fellow volunteers, and patrons of the organization as much as possible. I agree to utilize surgical masks or improvised masks such as scarves, bandanas, and handkerchiefs to reduce the risk of exposure to myself and others. I agree to wash or sanitize my hands after using the restroom, sneezing, coughing, using shared tools, and touching any high-touch surfaces. I attest that I am not experiencing any symptoms of illness such as a fever, cough, or shortness of breath, nor has anyone in my household experienced these symptoms in the last 3 days. If I develop these symptoms, I agree that I will notify LNC staff, cancel my volunteer shift, and stay home to limit the spread of infection. I understand that there is no direct medical health coverage afforded to me during my relationship with the Louisville Nature Center. The Louisville Nature Center is not responsible for any potential exposure to Novel Coronavirus, or COVID-19, even if caused by negligence on the part of their employees, volunteers, or the organization. By signing below, I agree to comply with the written instructions above. Failure to comply with these written instructions or verbal instructions from staff may result in my volunteer privileges being removed and I may be asked to leave the premises.
